Outstanding Service Award
The Mon Yough Fire Defense Council is looking for nominations for an Outstanding Service Award. We are looking for someone who has absolutely gone above and beyond the call of duty in the Fire, Rescue, EMS or any field a member company is involved in. Individuals can be nominated by any member of a department in the Mon Yough Fire Defense Council. If you feel you know of such a person or persons who have demonstrated Outstanding Service, please complete the application below. Use the back and/or additional paper as needed.
